Chronic Care Management
Developed on a proprietary survey engine, the system generates a set of targeted questions, customize to the patient’s specific history and state of health. Neutrino utilizes an easy to use mobile app, available for Apple and android smart phones and tablets.
Using predictive analytics based on the patient’s daily entries, the Neutrino engine through its unique C-score system detects significant changes. Any anomalies are pre-sorted on the web portal for easy identification.
Neutrino’s software collects and documents the data, in a PDF for both RPM and CCM. The software also establishes appropriate CMS billing codes
